Ghaemi and Goodwin's Bipolar

This is an extremely important article, for two reasons. First, the authors include two of the world's most respected bipolar authorities; and secondly, it completely changes the range of what is possibly "bipolar".
Thus this paper represents a "paradigm shift" of major proportions. Unfortunately, the online version from the publisher which used to be available (full text) can no longer be accessed. I even wrote the Canadian Journal of Psychiatry to ask about
this. Having no consistent success, I have taken the liberty of linking a "pdf" of the article (this is a copyright violation and I will take it down if anyone complains, noting here who the complainant was; but meanwhile, did not copy or redistribute this file).
